Accident On 281- Three-Vehicle Crash Injures Three, Two Children Seriously Hurt

A major crash on U.S. Highway 281 near Bitters Road left three people injured Saturday afternoon, August 15, 2026, after a driver reportedly suffered a medical episode and lost control of a vehicle.

The collision happened shortly before 3:30 p.m. on the northbound side of U.S. 281. According to the San Antonio Police Department, the incident involved three vehicles and resulted in serious injuries to two children.

Police said the driver of a Toyota Highlander experienced a medical episode on the frontage road, causing the vehicle to lose control. The Highlander reportedly went through a highway sign before colliding with a Dodge Ram and a Chevrolet Suburban.

The Dodge Ram rolled several times in the crash. Two children, ages 11 and 15, suffered life-threatening injuries and were transported to a hospital. The 40-year-old driver of the pickup also sustained injuries that were described as non-life-threatening.

The crash prompted authorities to shut down the northbound main lanes of U.S. 281 while emergency crews responded and investigators worked at the scene. The closure caused significant traffic congestion in the area, with motorists being diverted before Bitters Road.

By approximately 6:30 p.m. Saturday, the northbound main lanes had reopened, although the investigation remained active.

San Antonio police continue to investigate the circumstances surrounding the collision. The reported medical episode is a key part of the preliminary account, but authorities have not released additional details explaining what caused the driver to lose control.

The crash serves as another reminder of the dangers that can develop suddenly on heavily traveled highways. Emergency responders urged motorists approaching the scene to exercise caution and give first responders room to work.
